Youth Workshop Sign Up - Bring Your Own Improv

Youth Collective Registration Form

The program runs Fridays during your assigned session from 6:30pm to 9:00pm at the Warwick Center for the Arts, 3259 Post Road Warwick, RI 02886. Teens should dress comfortably, wear sneakers (no open-toed shoes allowed), and bring water.

Release Agreement for Parents of Youth
I understand that students under 16 years must be accompanied into the facility by a parent, guardian, or assigned contact when they are brought to class. A parent, guardian, or assigned contact must also physically come in to pick up their youth. Students must be picked up promptly at the end of class.

I agree to the fullest extent to be responsible for any medical bills which may incur resulting from illness or injury during my youth’s participation in Bring Your Own Improv’s activities. I also understand and agree that I am expected to carry my own accident and medical insurance. I release the instructor of the classes from any and all liability and/or claims or damages arising out of personal injury of any kind. If necessary, I authorize the instructor, to administer first aid and/or authorize medical treatment for my youth.

I give the program permission to utilize photographs, video, or audio of my youth for Bring Your Own Improv’s educational or promotional material.

I release to Love Thy Job, Bring Your Own Improv & The Warwick Center for the Art all rights to exhibit this work publicly or privately, including posting it on Bring Your Own Improv or The Warwick Center for the Arts’s websites and social media. I waive any rights, claims, or interest I may have to control the use of the material and agree that any uses described herein may be made without compensation or additional consideration of me; thereby waiving any right to inspect or approve the finished photograph or advertising copy for the eventual use that it might be applied.
